Consent to accept all Risks associated with ILEAP Activities

I hereby agree to safely deliver and collect the participant to and from their activity at the agreed drop off and pick up times and locations.

I agree that if I have any concerns over the setting for the activity or the activity itself, I will make an appointment to view the setting and/or the activity before the participant attends and I will contact ILEAP to liaise with them regarding the Participants safety whilst taking part in the activity at the specified venue.

In light of the Coronavirus pandemic, I will follow Government Guidance and the ILEAP COVID 19 Code of Conduct, to help to stop the spread of the virus and to protect my peer group, ILEAP Staff Volunteers and the wider community from the disease. If ILEAP Staff feel that I am not able to do this then I understand that I will not be able to attend ILEAP activities until it is safe to do so and that future booked sessions will be refunded

I assume full responsibility for any risk associated with taking part in the activities provided by ILEAP. I fully warrant ILEAP against any claim of any kind in relation to any loss, or injury however occurred.

Payment and Cancellation Policy

1. What happens if you have to cancel your sessions.

• For Group Activities and Day Trips: If for any reason you are unable to attend your pre-booked chosen activities you must notify us at least 7 days in advance so that we can reallocate You will be reimbursed with a free session (s) accordingly. 2. What happens if ILEAP have to cancel our sessions.

Occasionally due to lack of participants, bad weather, staffing issues or problems with venues we may have to cancel sessions. We will try and offer as much notice as possible and offer a full refund, which can be used towards future sessions.

3. Our Charges for Members

There is an annual subscription of £10 per year to join the Charity. Members are entitled to discounts at our group sessions and can receive 1:1 support. The membership fee will cover the period for 12 months from when you join. This fee should be paid when you join and once paid you will be able to book activities via our website

• The majority of our group sessions cost just £6 per hour and include support on a ratio of 5 participants: 1 member of staff / volunteer

• If you require ILEAP 1:1 support at your chosen activities then our 1:1 rate (£18 p/hr) will apply (the group fee will be waived).

If you require 1:1 support and you supply your own DBS checked worker then we can offer you, up to a 10% reduction on some of our group rates.

(Subsidise may be available, depending upon your age, where you live, your disability and our current grants and funding. We will discuss this with you on receipt of your registration form.)

• Members who request transport to and from their chosen activity with an ILEAP member of Staff will be charged a fee of £0.50p per mile for this service, plus Staff

4. When you will be charged

7. What will happen if payment has not been received

In the event of an outstanding payment the following will apply.

• Members who have not paid for their previous term time activities will not be eligible to attend the next block of term time or holiday

• Members who have not paid for their holiday activities will not be eligible to attend future holiday or term time
